Patient Information

CORE Physicians believe in continuity of care and making health care convenient. That means we provide care:

for those who want to see a doctor on the weekend.
for your out-of-town guests who need a doctor.
for those who don’t have a regular doctor.
for those whom their regular doctor is not available (we’re glad to send a prompt report to them).
as an outpatient
while in a hospital and/or nursing home.

The key to a good patient-physician relationship is open and honest communication. We encourage you to feel free to ask questions. It helps us both to avoid misunderstandings.

We are interested in the opinions of our patients. We encourage you to help us make your visit to CORE Physicians a pleasant one. If you feel we can improve our service, please call us at 931.388.9706.

Appointments

We see patients by appointments but have a Walk-In Clinic as well. We strive to see patients in a timely manner. At times, however, your physician may have to be away seeing other ill patients in the hospital or tending to an emergency. We ask for your understanding if this happens and will keep you informed of delays. If such delays occur, other arrangements can be made.

Insurance

We accept most insurance.
On your first visit, please bring another form of ID in addition your insurance card such as a drivers license or social security card.
Bring your insurance card on every visit.
We will file your insurance claim for you but you will be responsible for the bill.
If you have a secondary insurance plan, we will file it once
You are responsible for any amount your insurance company determines as “patient responsibility.”
If you do not have insurance, you will be asked to pay at the first visit.

Medical Records Release Form

This form is necessary if you want your medical records sent to an outside clinic or doctor or to have CORE Physicians obtain your records from an outside doctor or clinic. Click the link above and a pdf file will download on your computer which you can print out, complete, and either mail it to our office or fax it.
